The report states the witness saw the helicopter in an increasingly rapid descent before it disappeared behind buildings. He drove toward smoke and saw the helicopter was on fire in the parking lot of St. Ann Retirement Center. He assisted St. Ann employees in pulling the surviving paramedic away from the burning aircraft.

Rogers said that at first he couldn't tell that it was a helicopter. “ ... then it started going down ... I just had a weird feeling in my stomach and I saw smoke. When I got between two buildings, I could see a pretty good fire. It had landed about 15 feet in front of some gal sitting in an SUV eating her breakfast,” he said.

“I really didn't think the guy was going to survive. I really didn't,” he said, of the paramedic, Wynne.

Surveillance cameras located on a building adjacent to the parking lot showed the last few seconds of the helicopter descending toward the ground. The video showed the helicopter burst into flames upon impact.
