An online security breach at an Ohio-based insurance company may have revealed personal information of 534 Oklahomans, state Insurance Department officials said.

The FBI is investigating an Oct. 3 attack on the computer network of Nationwide Insurance Co., which is based in Columbus, Ohio.

The breach may have revealed some combination of customers' Social Security number, driver's license number and their birth date. Other possible information that may have been disclosed included marital status, gender, occupation and the employer's name and address.

Nationwide Insurance has begun sending notification letters to those whose information may have been compromised.

The company has opened a toll-free hotline to field questions at (800) 760-1125.
