Refinery officials, feds probe 2012 fatal explosion

WYNNEWOOD -- An internal investigation by officials at a Wynnewood refinery and that by the federal government are consistent, company officials said.



Two workers died of injuries suffered during the Sept. 28, 2012, explosion at Wynnewood Refining Company.

The company issued a statement that corresponds to Occupational Safety and Health Administration findings, the Pauls Valley Daily Democrat reports.

OSHA issued the refinery six citations; four of which were classified as serious and two were classified as other than serious.
